Pharmacists frequently encounter patients grappling with SDOH-related challenges, such as limited health literacy, transportation barriers, food insecurity, and inadequate access to healthcare services. These challenges not only compromise patient health but also pose significant barriers to achieving optimal therapeutic outcomes.Integration of SDOH into Pharmacy ServicesAchieving health equity and addressing healthcare disparities necessitates a systemic integration of SDOH considerations into pharmacy practice. The following detailed strategies outline how pharmacy professionals can proactively address SDOH-related challenges:Comprehensive Patient Assessment-Develop standardized screening tools to identify SDOH-related factors impacting patients' health.- Utilize electronic health records to systemically document SDOH information and facilitate data-driven interventions.
