New value-based care company tackles health disparities among rural seniors in America.

Co-founders Brian Mathis and Kari Severson Snaza have come up with Oasis Health Partners, a new firm focusing on value-based care in rural America. The company is dedicated to creating healthier communities by increasing elder primary care.
"The platform we have developed enables primary care providers to access patient-specific reporting, improve their workflow, spend more time with the patients who need it most, and receive incentives based on the overall health of their patients. This team and platform, combined with our strong partnerships with health plans, will allow us to break down barriers to high-quality preventative care and build healthier communities," adds Mathis.
More than 10,000 Americans become eligible for Medicare every day. A quarter of these individuals reside in rural areas where access to preventative treatment might be difficult, leading to a higher prevalence of chronic diseases, higher mortality rates, and other health disparities.
